Insurance

Services are provided in-network for the following insurance plans:

• Wellmark Blue Cross Blue Shield
• Blue Cross Blue Shield plans
• Optum 
• United Behavioral Health 
• UMR

If you are covered under another insurance plan, it is possible services can be provided out of network. Upon request, you will be provided with an itemized bill (superbill) that you can submit to your insurance company for possible reimbursement 

Good Faith Estimate for Services

Under Section 2799B-6 of the Public Health Service Act (No Surprises Act), health care providers are required to offer a Good Faith Estimate of expected charges for clients who are not using insurance or other health care benefits to pay for services. If you are self-pay or not using insurance, you can request a Good Faith Estimate at any time and you will be provided with a written estimate of the costs for your counseling services. 

Payment

Cash, check, debit cards, and all major credit cards are accepted as forms of payment. Co-payments, co-insurance, and payments toward deductibles are due at the time of service.

Cancellation Policy

If you are unable to attend a session, please cancel at least 24 hours in advance. Cancellations with less than 24 hours’ notice, or missed appointments, may be charged a cancellation fee.
